What is a manic episode?

manic episode: distinct period of abnormally and persistently elevated, expansive, or irritable mood and increased energy and activity. Lasting at least 1 week, present most of the day, nearly every day. At least three or more symptoms (4 when only irritable mood)
  • Inflated self-esteem or grandiosity
  • Decreased need for sleep
  • More talkative
  • Racing thoughts or flight of ideas
  • Distractibility
  • increase in goal-directed activity or psychomotor agitation (purposeless)
  • excessive involvement in activities that have high potential for painful consequences. Marked impairment in social or occupational functioning, or hospitalization. Psychotic features possible

What is a hypomanic episode?

distinct period of abnormally and persistently elevated, expansive, or irritable mood and increased energy and activity. Lasting at least 4 consecutive days.
  • Same symptoms as manic disorder 
  • The disturbance in mood and the change in functioning are observable by others. The episode is not severe enough to cause marked impairment. Not hospitalized. Psychotic features not possible.

Note: Hypomanic episode is common in Bipolar I, but not required for diagnosis.

What is criteria A for bipolar II disorder?

at least one hypomanic episode and at least one major depressive episode. But there has never been a manic episode.

What is criteria A for cyclothymic disorder?

For at least 2 years (1 year in children and adolescents) there have been numerous periods with hypomanic symptoms but do not meet the criteria for a hypomanic episode. And numerous periods with depressive symptoms that do not meet the criteria for depressive episode.

What is criteria B for cyclothymic disorder?

Symptoms of hypomanic or major depressive eposide have been present for at least ½ of the time.
