When must a trip report be filled in?

When must a ASR be filled in?
An ASR shall be filled in to report all safety related events and safety hazards/concerns during the operation of flight. In those cases a trip report is NOT needed anymore

Who can fill out an ASR?
Usually the commander will fill out an ASR, but if for any reason he declares he will not then CA's should do this, before submitting an ASR the commander of the specific flight must be informed.
