Heart failure - Blood pressure control

7 important questions on Heart failure - Blood pressure control

What is the myogenic response of autoregulation?

Bayliss myogenic response
=> stretching of vascular smooth muscle cells causes depolarizatoin of SMCs > contraction > reduction of vascular diameter.

This mechanisms counteracts pressure induced changes in vascular diameter. Therefore the pressure in capillary remains the same.

Why is there also extrinsic control of blood vessels?

To regulate overall SVR and control mean arterial bp
To allow the brain to alter blood flow selectively to individual organs

Which receptors and effects are important in neural control of the vessels?

Sympathetic = noradrenaline
Parasympathetic = ACh

NA binds to alfa receptors on bv which causes vasoconstriction or to ß receptors which causes vasodilatation
ACh binds to M2 receptors -> vasodilatation

What is the effect of NAd and ACh on the heart?

NAd binds to ß and alfa receptors - increased inotropy, chronotropy, dromotropy
ACh - decreased inotropy, chronotropy and dromotropy & inhibits release of NAd

When is adrenaline produced and by which organ?

Produced by the adrenal medulla under sympathetic stimulation.

How does the RAAS system work?

Sympathetic/hypotension/decreased Na delivery result in signals to the kidney and release renin. Renin converts angiotensinogen into AngI. ACE (surface lungs) converts AngI to Ang II. AngII has several effects

What is the primary role of baroreceptors?

  • Blood flow to the brain & myocardium must be maintained at all costs.
  • It is essential, therefore, to maintain arterial blood pressure in the carotid and coronary arteries.
  • Blood pressure sensors have evolved in the walls of the carotid arteries and aorta, to inform the brain of the pressure in these key feeder vessels.
  • The sensors, BARORECEPTORS, actually detect wall stretch, which increases with pressure
